We are looking for a compassionate and skilled Case Manager to join our team and make a difference in the lives of our clients. Job Summary: The Case Manager will play a crucial role in coordinating and providing care for clients undergoing substance abuse treatment. This position involves developing individualized care plans, monitoring client progress, and facilitating access to necessary services and resources. The ideal candidate will have strong organizational skills, excellent communication abilities, and a deep commitment to helping individuals achieve and maintain sobriety. Key Responsibilities: Conduct comprehensive assessments to identify clients’ needs, strengths, and treatment goals. Develop and implement individualized care plans in collaboration with clients and the treatment team. Monitor and evaluate client progress, adjusting care plans as needed to ensure optimal outcomes. Provide ongoing support and advocacy for clients, assisting them in navigating healthcare systems and accessing community resources. Facilitate group and individual counseling sessions as appropriate. Coordinate with medical, therapeutic, and support staff to ensure holistic care. Maintain accurate and timely documentation of client interactions and progress in accordance with facility policies and regulatory requirements. Assist clients in developing coping strategies and life skills to support their recovery. Educate clients and their families about substance abuse, treatment options, and available support services. Participate in staff meetings, training sessions, and continuing education opportunities to stay current with best practices in substance abuse treatment. Uphold client confidentiality and adhere to ethical standards in all interactions. Qualifications: CADC Certification or relevant degree Minimum of 2 years of experience in substance abuse treatment or a related field. Knowledge of addiction treatment modalities and community resources. Strong interpersonal and communication skills, with the ability to build rapport with clients and colleagues. Excellent organizational skills and the ability to manage multiple tasks simultaneously. Empathy, patience, and a non-judgmental attitude towards clients. Proficiency in electronic health records (EMR) systems and KIPU. Ability to work independently and as part of a team.
